How to Get a Death Certificate in Alabama
The State of Alabama issues death certificates to the general public for deaths occurring within the state more than 25 years ago. For deaths occurring less than 25 years ago, access is restricted by law. Provided the death certificate you request is for a death more than a quarter of a century ago, or if you are among those eligible to receive a copy of the record, you must apply directly to the Alabama Department of Health, or through a third-party processing center, for the certificate
Determine that you are among those eligible to receive a copy of the death certificate. For deaths occurring less than 25 years ago, Alabama law restricts access to the parents, spouse, children, grandchildren and siblings of the deceased. In addition, a legal representative of the family may also have access to the document.

Gather the required information. The Alabama Department of Public Health requests that applicants supply as much information as possible to aid in the search for the record. Therefore, you will be required to supply the full legal name, date and place of death, the sex, social security number, date of birth or age and race of the deceased. In addition, the name of the deceased's spouse and parents will be required. Finally, your name, your relationship to the deceased and your contact information, including where the certificate is to be mailed, will need to be supplied as well.

Download the application from the Alabama Department of Public Health website. Fill out as much as possible.

Mail in the application, along with a photocopy of your government-issued photo I.D and the appropriate fees. As of April 2010, the death certificate application fee was $15 for the first copy, or a "Certificate of Failure to Find," and $6 for each additional copy. The fees are payable in a check or money order made out to "Vital Records;" cash is not accepted.

Wait for the certificate to arrive. Processing can take several weeks.
